1. Sign and send the License Agreement

Select your License Agreement below and print it. Then read it, sign it and mail it to us. All the instructions are printed on the front page of the agreement.

2. Pay the setup fee

To pay by check, simply include your check with your License Agreement when you mail it to us.

To pay by credit card or PayPal, visit the payments page.

The setup fees are:  Clubs $95 | Trainers $65

3. Get setup

Once we receive your signed license agreement, we will email you instructions for getting started with FitnessAge.

4. Get trained

A FitnessAge rep will contact you to arrange a training appointment which will take place by phone and online.
